Sun River Health is committed to patient privacy. As a patient, you have the right to access your medical records.

In order to protect your privacy, only the patient, parent/legal guardian, or the patient’s legal representative can sign the Authorization for Use and Disclosure form to release medical records.

You can access your medical records the following ways:

Change request:
You can request a change in your medical records by any by dropping off a written request at any Sun River Health site. The request must be signed by the patient or legal guardian. You can also request the change

Frequently Asked Questions

  • Yourself (not a spouse)
  • A parent if the patient is under the age of 18 years of age
  • Court appointed guardian. (Must provide legal documents)
  • Power of Attorney if patient is unable to sign (must have documentation)
  • Executor/representative of estate for deceased patient with a copy of the short form (death certificate presented)
  • A minor may sign their own authorization if they are:
    • Emancipated
    • Pregnant
    • Being treated for venereal disease, sexual assault, alcohol or drug use/abuse
    • Age 14 or older and is admitted to a psychiatric facility, children’s crisis intervention service or special psychiatric hospital operated by a state-licensed mental health provider

NYS law prescribes a .75/page fee for medical records.

You will receive your medical records within 7 days of your request.
