Welcome to the Sun River Health Care Management program!  

We look forward to working with you to manage your health and make sure you receive the best care. 

We want to make sure you have all the information you need about your participation in this program, your rights, and how we will work together to make sure you get the care you need. 

Your care management services will be guided by a dedicated nurse care manager. You may request information on their qualifications, the services they offer, and any contracted relationships with our clinicians. Our goal is to make sure you always have the information you need. 

As a participant in the Care Management program, you have the right to: 

  • Respectful treatment from all Sun River Health staff 
  • Know who your care manager is and how to request a change in care manager if you would like 
  • The support you need to collaborate on decisions about your care 
  • Privacy and security so your personally identifiable data and medical information is kept confidential 
  • The choice to decline participation or disenroll from the program at any time 

To help us provide the best possible support, we ask you to: 

  • Work with us to follow your mutually agreed upon care management plan 
  • Give us any important personal or health information that affects your care 
  • Let your care manager know if you decide to leave the program
