My Healthy Masterpiece 

Sun River Health Youth Art Contest 

Guidelines and Submission Form 

Artists must be between the ages of 4 and 17 and a patient at either the Sun River Health Valentine Lane or Park Avenue. (New patients are welcome!) 

Instructions: Using the theme My Healthy Masterpiece, create a piece of art and provide a brief introduction to your piece on a separate sheet of paper. (Please print or type your introduction) 

Material:  

  • Artwork must be submitted on white paper at least 8.5” X 11” and no larger than 11”x17” 
  • Any 2D media can be used - crayons, markers, colored pencils, etc. 
  • Paper and crayons can be picked up at the Park Avenue or Valentine Lane Sun River Health Centers. Artists can use their own materials as long as they meet the criteria 
  • No digital submissions will be accepted (JPEGs, etc.); submissions must be on paper 

Deadline and Submissions:  

Contest submissions should be returned to a Park Avenue or Valentine Lane front desk staff member no later than Thursday, June 18th. Please make sure to include: 

  • Artwork 
  • Introduction -- please tell us about your artwork. Use a separate sheet of paper 
  • Consent form- gives permission to use artwork, introduction to art, photograph of artist 

  • Forms are available at your Yonkers Sun River Health center.

Pautas y formulario de presentación 

Los artistas deben tener entre 4 y 17 años y, además, deben ser pacientes de Sun River Health de Valentine Lane o Park Avenue. (¡Se aceptan nuevos pacientes!)  

Instrucciones: utilizando el tema My Healthy Masterpiece, crea una obra de arte y escribe una introducción corta a tu obra en una hoja separada. (Escribe tu introducción a mano en letra de imprenta o a máquina) 

Material:   

  • La obra debe presentarse en papel blanco de 8.5 pulg. X 11 pulg. como mínimo y 11 pulg. X 17 pulg. como máximo 
  • Puede usarse cualquier medio en 2D: crayones, marcadores, lápices de colores, etc.  
  • Se puede recoger papel y crayones en los centros de Sun River Health en Park Avenue o Valentine Lane. Los artistas pueden usar sus propios materiales siempre que cumplan los criterios 
  • No se aceptarán obras digitales (JPEG, etc.); las obras deben presentarse en papel 

Plazo y presentaciones:   

Las presentaciones para el concurso deben enviarse a un miembro del personal de recepción de Park Avenue o Valentine Lane a más tardar el jueves 18 de junio. No olvides incluir lo siguiente:  

  • La obra 
  • Introducción: cuéntanos sobre tu obra de arte. Usa una hoja de papel separada 
  • El formulario de consentimiento da permiso para usar la obra de arte, la introducción y la fotografía del artista 

  • Los formularios están disponibles en su centro Sun River Health de Yonkers.
